Measurements:
a. Total Height: 43 mm (4.3 cm)

Pileus/Cap: centrally depressed; areolate with dark green plaque-like patches; center is completely green, with areolate marks extending to the margin; areolate marks fade from green to a yellow-grey when approaching the margin; margin smooth and striated; cap smooth; dry; surface can peel about 2/3 from the margin to the center
Hymenium/Gills: gills white; very minutely decurrently attached; gill pattern consists of long, thin, compact blades that are of even length
Stipe/Stalk: central to the pileus; white, white the very bottom of the base yellowish-brown; thick; solid; equal
Flesh: thin to medium thickness; white; no color change upon exposure
Lactation: No
Odor: Not Distinctive
Chemical Reactions: negative with KOH and Ammonia on pileus; FeSO4 also negative on flesh
Spore Print: yellowish-white
Microscopic Observations: amyloid; subglobose to globose; spiny; thin walled; dotted; 6.5 x 7.5 micrometers

Hymenium: pored; vary in size; shallow; irregularly shaped; sinuate attachment; natural pale to golden yellow; does not stain when bruised
Stipe/Stalk: Granular-dotted; annulus is adhered to the stipe; no volva; granular dots are reddish toward the apex brownish purple toward the base; white background; solid interior; central
Flesh: pale yellow to white; does not stain blue upon exposure or when bruised; stains in KOH
Lactation: No
Odor: Not distinctive
Chemical Reactions: pileus stains greyish brown in KOH
Spore Print: Brown
Microscopic Observations: inamyloid; ellipsoidal to subfusoid; 8.75 x 2.5 micrometers

Hymenium/Gills: Gills run down the stalk (decurrent); dull orange-brown in color; gills even and tightly packet; thin-gilled
Stipe/Stalk: Same color as the gills; solid throughout; tapered downward

Lactation: Yes; taste was hot and acrid; color was a creamy white

Pileus/Cap: Brown-orange to orange to pale orange in color; convex to broadly convex; slightly depressed; dry; scaly/scurfy; margin striated with some bits of veil fragments.
Lamella/Gills: gills close; same color as the pileus; adnate to slightly decurrent.
Stalk/Stipe: same color are the pileus; equal all around; hollow; smooth; thin; 25-30 mm (2.5-3.0 cm) long; more or less central; annulus absent; white mycelium at the base.
Flesh: Thin and fragile
Spore Print: Rusty orange to ochre brown
Spores: 6.0-7.25 x 5.0-6.25 microns; smooth; elliptical; inamyloid; brown to dark orange in Meltzer's Iodine; minutely roughened; negative in KOH

Stalk/Stipe: yellow towards the apex; reddish-brown towards the base; tapered downward; no annulus or volva; central; fine reticulation towards the apex; bruises blue/black
Flesh: bruises and stains evenly into a very pale/light grayish-blue
Spore print: Dark olive to brown
KOH: brownish-orange on flesh
Spores: 5 ocular units (12.5 microns); smooth; thin-walled; ellipsoid to spindle-shaped to subfusoid; inamyloid; Golden in Meltzer’s Iodine
