Looks like four upper labials and two enlarged postlabials (page 308 in Peterson guide 4th edition), which eliminate Southeastern Five-lined and Broad-headed. The middle row of undertail scales doesn't look convincingly wide to me, but maybe it is the angle. Or maybe the tail has regenerated?
